Studi Banding Pemeriksaan Histopatologi Sediaan Slaid Potong Beku dan Blok Parafin pada Penderita Kanker Payudara di RSUP Dr. Kariadi Selama 3 Tahun (1995 – 1997)

Latar Belakang : Pemeriksaan sediaan slaid potong beku (SSPB) merupakan cara pemeriksaan yang sangat penting dilakukan untuk menentukan tindakan yang tepat pada saat operasi. Tindakan tersebut diikuti pemeriksaan sediaan slaid blok parafin (SSBP) . Kadang-kadnag ditemukan ketidaksesuaian antara diagnosis SSPB dan SSBP, sehingga dapat menimbulkan kesulitan terapi.

Tujuan : Mengatahui seberapa jauh pemeriksaan SSPB dapat membedakan kelainan keganasan atau bukan keganasan, serta tingkat akurasinya.

Rancangan : Uji diagnostik

Tempat : Bagian Patologi Anatomi FK UNDIP / RSUP Dr. Kariadi Semarang

Subyek dan Metode : Semua formulir pengiriman sediaan penderita tumor payudara, SSPB dan SSBP selama 3 tahun (1995 – 1997) direevaluasi, dicatat serta dibuat tabel. Setelah itu dilakukan uji diagnostik dengan menggunakan SSBP sebagai baku emas, serta dilakukan uji statistik dengan tes X2.

Hasil : Dari 378 kasus tumor payudara yang diperiksa, didapat 61 kasus tumor ganas. Sisanya 317 kasus merupaka tumor jinak. Kanker payudara paling banyak berkisar antara umur 35 – 54 tahun. Gambaran mikroskopik terbanyak adalah karsinoma duktus invasif >(72.49%). Sensitivitas : 91.80%, spesifisitas : 98.73%, nilai duga positif : 93.33%, nilai duga negatif : 98.40%, akurasi : 97.61%.

Kesimpulan : Pemeriksaan SSPB sangat peka dalam membedakan keganasan atau bukan keganasan, serta hasilnya akurat.

Background : The examination of frozen section slides is an important method for defining accurate procedure during breast surgery. The examination is always followed by examination of paraffine block slides. Sometimes there are still unmatching between frozen section slide and paraffine block slide in diagnosing breast cancer, so it cause difficulties in therapy.

Objective : To know how far the examination of frozen section slides can differentiate between malignancy and benign disease, and to evaluate its accuracy.

Design : Diagnostic test.

Setting : Department of Pathology Anatomi Faculty of Medicine, Diponegoro University / Dr. Kariadi Hospital, Semarang.

Subject and Method : All registration from of breast tumour patients, frozen section and paraffine block slides during 3 years (1995 – 1997) were reevaluated, recorded and tablet. After that the datas are performed diagnostic test by using parffine block slides as gold standard and statistical test by X2 test.

Results : among 378 of breast tumor cases, consisted of 61 malignant tumours and 317 benign cases. Mostly the age of patient suffering from breast cancer were between 35 – 45 years. Microscopic features were mostly invasif duct carcinoma (72.49%). Sensitivity : 91.80%, specificity : 98.73%, positive predictive value : 93.33%, negative predictive value : 98.40%, accuracy value : 97.61%.

Conclusion : The examination of frozen section slides is very sensitive and very accurate for detecting malignancy.
