Kecenderungan Nyeri Kepala Tegang pada Kelompok Individu dengan Sistem Saraf Hipereksitabel

Objektif: Untuk mengetahui odds ratio nyeri kepala tegang di antara kelompok dengan saraf hipereksitabel dan normal
Desain: Studi kasus kontrol. Pasien dan Metode: Dan poliklinik saraf Rs Dr. Kariadi Semarang dipilih secara konsekutif 198 pasien yang memenuhi inklusi dan eksklusi, masing-masing untuk kelompok studi dan kontrol, selama tahun 1998. Pada tiap pasien dilakukan tes provokasi menggunakan alat EMG untuk menilai eksatabilitas sarafnya, atas dasar awitan gelombang potensial duplet yang muncul dilayar, frekuensi gelombang repetitif per detik dan durasi gelombang repetitif ini sebelum menghilang sama sekali.
Hasil: Kelompok kontrol terdiri atas berbagai diagnosis penyakit saraf antara lain sindroma terowongami karpal (20.2%,), sindromia servikal (l0.6% iskhialgia (16.2%) dan mialgia (10.1%). Odds ratio untuk nyeri kepala tegang pada pria (14-25 th) adalah 1.67 (95% Cl 0.80,27.08) dibandingkan dengan pada wanita . 4.1(95% CI 1.29:13.00). Pada usia lebih tua (45-60 years), odds ratio pada pria 0.7 (95% CI 0.14:3.56) dan 2.29 (95% Cl 0.77:6.83) pada wanita. Proporsi wanita dengan hiperreksitabilitas saraf (usia antara 45—60 tahun,) lebih besar secara bermakna dibandingkan pria ( 02). Ditinjau dari kelompok nyeri kepala tegang, proporsi individu dengan saraf hiperexsitabel lebih tinggi secara bermakna dibandingkan tipe kronik (p=0, 008).
Kesimpulan: Kelompok individu dengami saraf hipereksitabel cenderung untuk menderita nyeri kepala tegang dengan peluang kurang lebih 4 kali dibandingkan orang normal. Kecenderungan ini berkurang sejalan dengan bertambahnya usia (generalisasi hospital based)
Objective: To estimate the odds ratio of tension headache among people with and with nerve system hyperexcitability..
Design: A case control design was conducted for this study.
Patients and methods : three hundred and ninety six cases (age 14-60 years,) were recruited from the neurology outpatient clinic of Dr Kariadi hospital in Semarang during the year 1998, who met the inclusion and exclusion criteria either for case or control group. Provocation test was conducted to all of them using the EMG apparatus (Amplaid), to evaluate their nerve excitability based on time on set of repetitive duplet or multiplet wave potentials emerging on the screen, the frequency of the potential per second and their repetitive duration before subsiding.
Result: The control group consisted of variety of diagnosis, among them were carpal tunnel syndrome (‘20.2%), Cervical syndrome (10.6%) ischialgia (16.2%) and mialgia (10.1%).The odds ratio for tension headache in men (age 14-25 years,) was 4,67 (95% CI 080.27.08) compared to women ; 4.1 (95% CI 1.29,13.00). in the older age group (45-60 years,), The odds ratio for men was 0,7 (95% C10.14,3.56) and 2.29 (95% Cl0. 77:6.83) for women. ihe proportion of women with nerve hyperexcitability (age range 45-60) years) was significantly higher than men ( p=0.02) The chronic tension headache group had a higher proportion of nerve system hyperexcitability compared with the episodic tension headache ( p=0,008,).
conclusion: People with nervehyperexcitability has the chance to experience tension headache approximately 4 times compared those with normal nerve excitability. The tendency is decreasing along with increasing age (‘hospital based generalization).
