EEG pada Individu dengan Spasmofili

Latar Belakang: Individu dengan spasmophilia (syndrome . Khronik) menunjukkan hiper-eksibilitas saraf tepi secara elektrofisiologic,Di pihak lain peneliti – peneliti menemukan adanya hubungan antara spamophilia dengan beberapa gangguan sentral otak seperti syndrome panik, sindroma ansietas maupun fobia .Adakah hypereksitabilitas sentral juga pada spasmophilia?

Objektif : Membuktikan bahwa pravelensi EEG iritatif pada kelompok spamophilia lebih tinggi secara bermakna dibandingkan non spasmofili . Desain: Studi belah lintang dengan seting rumah sakit rujukan Pasien dan desain : Pasien yang dikirim untuk pemeriksaan EEG ke RS.Dr.Kariadi dan memenuhikriteria inklusi serta eksklusi diikutkan dalam penelitian ini .Spasmophilia ditentukan dengan cara tes provokasi EMG .Derajat spasmophili ditentukan atas dasara omset dan frekuensi gelombang multiplet yang direkam dari m.abductor pollicis brevis.Rekaman EEG baik sewaktu istirahat maupun hiperventialsi dibuat pada semua kasus.Angka Kappa untuk inter –rater agrement pada interpelasi EEG adalah 70% .Beberapa parameters EEG dikumpulkan sebagai data untuk masing masing kasus penelitian.
 
Hasil : 100 kasus yang berhasil dikumpulkan yang 67% Diantaranya adalah wanita dan 65% Dengan spasmophilia . Dari kelompok dengan spasmophilia 73,2% adalah wanita .Tiga parameter EEG yang dperoleh dari rekaman hyperventilasi , menunjukkan korelasi yang relatif kuat dengan spasmofili :

1)peningakatan frekuensi gelombang tajam/runcing (Spearman rho 0,41 ; p=0.002)
2). peningkatan amplitudo gelombang tajam dan runcing (Spearman rho 0,27 : p=0,007)
3)Peningkatan frekuensi gelombang paroksimal lambat (Spearman rho 0,20 : p=0.05). Rasio pravelens kedua parameter EEG yang lain adalah :

1) adanya gelombang tajam/runcing : 2.34 (95% CI 0,89-6,17 dan
2)adanya gelombang paroksimal lambat beramplitudo tinggi (50µV ) : 3.40 (95% CI 1.10-10.55)

Kesimpulan :
Kelompok spasmophili mempunyai kecendurungan menunjukkan baik Hypereksibilitas sistem saraf tepi maupun sentral (serebal).

Background: People with spasmphilia(Chronic hypersensitilation syndrome)electrophysiologicslly display peripheral nerve hyperexcitabillity.On the other hand spasmophilia was related with severaal central disorder such as panick syndrome ,anxiety syndrome and phobia.Is there also a central or celebral hyperexcitability in spasmophilia. Objective : To demonstrate whether the prevalence of irritable EEG is significantly higher in the spasmophilia group compare with non-spasmophilia group Desain : Croos sectional study with reverral hospital based setting. Patients and

Methods: Patients sent to RS Dr.Kariadi for EMG examination which met the inclusion and exclusion citeria were included in the study. Spasmophilia was determined by EMG provocation test.Grading of spasmophilia was based on the omset and frequensy of multiplet waves,recorded from abductor pollicis brevis muscle. EEG recording (at rest and with hyperventilation)were taken from all patients.Inter – rter agrement (kappa value)of the EEG interpretation was 70% .Several EEG parameters taken at rest and eith hyperventilasion were gathered for each patients.

Result : Hundred patients were collected , among them 67% were women and 65% were spasmophilia.Among the spasmophilic cases 73,2% were women .Three EEG parameters taken with provocation technique (hyperventilation)displayed a relative strong correlation with spasmophilic condition : 1) . Increased frequency of sharp or spike waves (Spearman rho 0,41 : p=0,00) 2). increased amplitude of sharp or spike waves (Spearman rho 0,27 : p=0,007) 3). increased frequency of paroxysmal slow waves (Spearman rho 0,20 : p=0,05). Ratio prevalence of the other two EEG parameters : 1)the existence of sharp or spike waves :2.34 (95% CI 0,89 – 6,17 and 2) .the existence of relative high amplitude (50µV ) and paroxysmal slow waves :3.40 (95% CI 1.10-10.55)

Conclusion : Group with spasmophilia have the prospensiy to dispaly both hyperexcitability of the peripheral nerve and central nerve system (cerebal).
